Learning Disabilities Team
Working with joint teams in commissioning from Norfolk Community Health and Care (NCHC) our Learning Disabilities team brings together social workers, nurses and other specialists to wrap care around the person.
Mental Health Team
Working alongside health partners our mental health teams work jointly to share information providing an integrated person centred approach.
Our Community Care team provides support and promotes independence and well-being for older people and vulnerable adults across Norfolk.
Emergency Duty Team
Our Emergency Duty Team (EDT) provides emergency out of office support for people who are either in danger, unable to protect themselves or for people unable to remain in the community without immediate intervention.
